In short: Your immune system relies on a precise choreography of cell signals. At the center of this dance is a single master regulator: the class II transactivator. Without it, your body loses its ability to recognize external threats, leading to severe immunodeficiency.

MHC class II molecules are essential components of the adaptive immune system. They act as specialized receptors on the surface of professional antigen-presenting cells—such as macrophages, dendritic cells, and B cells—to display fragments of pathogens like bacteria to helper T cells. This process is vital for initiating protective immune responses and maintaining the balance of the immune population.

Because these molecules must be expressed in a strictly controlled, cell-type-specific manner, their regulation is governed almost exclusively by one factor: the class II transactivator (CIITA). Ciita functions as a non-DNA-binding co-activator that dictates where and when MHC class II genes are turned on. Its own expression is managed by three independent promoters with distinct roles.

Promoters pI and pIII drive expression in myeloid cells (like macrophages) and lymphoid cells (like B cells), respectively. Meanwhile, promoter pIV is uniquely responsible for driving expression in thymic epithelial cells—which is necessary for T-cell development—and in non-haematopoietic cells stimulated by interferon-γ. When this regulatory machinery fails, the consequences are severe. Mutations in the CIITA gene cause bare lymphocyte syndrome (BLS), a human immunodeficiency disorder characterized by a global loss of MHC class II expression. This breakdown prevents the body from properly presenting antigens, leaving it vulnerable to infection. Understanding these specific promoters allows researchers to explore new therapeutic strategies for modulating how the body presents antigens during disease.
