How Sandy Alcántara transformed from a Dominican prospect into a historic MLB strikeout leader
In short: Born in the Dominican Republic, Sandy Alcántara rose from humble beginnings to become a dominant force in Major League Baseball. From his unanimous Cy Young Award win to overcoming major elbow surgery, his career is a testament to resilience, evolving from a raw talent into a record-breaking franchise cornerstone.
Born in San Juan de la Maguana in 1995, Alcántara moved to Santo Domingo at age 11 to pursue baseball, eventually dropping out of school in eighth grade to focus entirely on the sport. Signed by the St. Louis Cardinals in 2013, he climbed through the minor leagues before being traded to the Miami Marlins in 2017. His breakthrough arrived in 2022, when he posted a league-leading 8.0 bWAR and unanimously secured the National League Cy Young Award, cementing his status as one of the game's elite pitchers.
The trajectory of his career was interrupted in 2023 by a ulnar collateral ligament sprain, necessitating Tommy John surgery that sidelined him for the entire 2024 season. Returning in 2025, Alcántara faced significant struggles, including a mid-season ERA that climbed to 7.14. Yet, he displayed characteristic grit, stabilizing his performance to finish the year with an 11–12 record and re-establishing his leadership role within the Marlins clubhouse.
By 2026, Alcántara had fully reclaimed his form, setting a new Marlins club record with his 1,002nd career strikeout. That same season, he achieved a rare feat by recording a save while leading the league in innings pitched, a dual accomplishment not seen since Orel Hershiser in 1987. Beyond his on-field achievements, Alcántara has remained committed to his roots, partnering with The Giving Much More Foundation to provide equipment to underprivileged youth in the Dominican Republic, ensuring his impact extends well beyond the diamond.
