In short: From a crushing 11–0 loss in 1976 to historic wins on the road, the São Tomé and Príncipe national team has endured a volatile journey. Their history is defined by long hiatuses, sudden ranking surges, and a dramatic legal battle to remain in continental competition.

The footballing history of São Tomé and Príncipe began in May 1976 with a 6–1 friendly loss to Gabon. The debut year was difficult; during the 1976 Central African Games, the team suffered an 11–0 defeat to Congo, their largest loss to date. They also suffered losses to the Central African Republic and Chad. It was not until 1977 that the squad secured its first victory, defeating Rwanda in a friendly match.

The team's trajectory has been marked by significant gaps in activity. Following an eleven-year hiatus, they entered their first FIFA-sanctioned tournament at the 1999 UNIFAC Cup, securing wins against Equatorial Guinea and Sierra Leone. This period of success propelled them to an all-time high FIFA ranking of 179. However, subsequent struggles and a four-year period of inactivity—starting with a withdrawal from 2010 World Cup qualifying—saw them drop out of the FIFA rankings entirely.

Recent years have seen a mixture of breakthrough successes and administrative drama. In 2012, the team achieved its first aggregate win by defeating Lesotho. A significant milestone occurred on 9 October 2019, when they secured their first competitive away win with a 1–3 victory over Mauritius. Most recently, during the 2023 AFCON qualifiers, the team faced a legal crisis when CAF awarded Mauritius a 3–0 victory due to a COVID protocol violation involving player Luis Leal. After an initial unsuccessful appeal, São Tomé was reinstated by CAF just one week before their first qualifying group game.
