In short: In The Darkness II, mafia hitman Jackie Estacado battles for his soul and his girlfriend's. Armed with supernatural Demon Arms and a goblin-like Darkling, he must navigate a surreal reality where the line between a violent mob war and a psychiatric delusion is dangerously thin.

Released in February 2012 by publisher 2K, The Darkness II is a first-person shooter developed by Digital Extremes. The sequel to the 2007 title follows Jackie Estacado, a mafia hitman who possesses an ancient demonic force known as "the Darkness" within his bloodline. To navigate the narrative, the game utilizes a "graphic noir" art style, with many assets hand-painted by artists to mimic the high-contrast aesthetic of the original Top Cow Productions comic books.

The gameplay blends traditional shooter mechanics with role-playing elements, including skill trees divided into four aspects: Hitman, Execution, Darkness Powers, and Demon Arm. Players can use supernatural abilities like "Black Hole" or "Swarm," and utilize Demon Arms—tentacle-like appendages that can devour fallen enemies for health. A vital feature is the Darkling, a goblin-esque entity that aids during battle and can be steered through narrow gaps.

The story is structured as a three-act journey that transitions from a survival-focused mob war into a personal love story. Jackie faces the Brotherhood, a secret society of cultists attempting to seize his power using a device called the Siphon. The narrative is famously surreal, frequently shifting between a violent urban conflict and a psychiatric ward where Jackie's crew appear as patients and staff, leaving the player to question if the mobster's life is merely a delusion.

While the game received positive reviews for its pacing and art, critics noted its short length and technical issues. Despite these flaws, it achieved commercial success, debuting as the third best-selling game during its first week of release in the United Kingdom.
